A 52 year old male patient  came to the causality with the complaint of fever and decreased  appetite since 10 days
HISTORY OF PRESENT  ILLNESS  :
A 52 yr old male patient who is farmer by occupation  was apparently asymptomatic 1month ago then he developed fever which is continuous  and  had vomitings  after taking food and had weakness and decreased  appetite  and complains of hiccups which are intermittent (2days) 
 HISTORY  PAST ILLNESS :
No h/o  asthma, epilepsy,  tb, diabetes,  thyroid , hypertension ,  cardiac problems
PERSONAL HISTORY :
Married 
Diet : mixed
Appetite : decreased  
Allergies : no
Sleep : inadequate 
Bowel movements : irregular 
Micturation : normal
Addiction : he used take alchohol but stopped  taking alcohol  from 2months
FAMILY  HISTORY  :
No relevant family history 
GENERAL  EXAMINATION  :
Patient is conscious , coherent,  cooperative and well oriented to time and place
Hlo pallor
No history Clubbing,  cyanosis 
Lymphadenopathy, icterus ,edema

BP: 110/80
SYSTEMIC  EXAMINATION :
CVS  : no thrills,no cardiac murmurs
Cardiac sounds : s1 s2 +
RS :  no dysponea $ wheeze
Shape of abdomen  : scaphoid
CNS : conscious,  ,Coherent , no stiffness  of neck
DIAGNOSIS : 
Iron deficiency  anemia 
INVESTIGATIONS : 
ESR : 60 ( nrml : 3- 15)
Random Blood sugar : 106 ( nrml :100- 160)
Hemogram : 
Decreased  Hb ,Pcv, Rbc
complete urine examination 
ECG 
Ultrasound  :
H/o pallor
