A 14 yr old male patient with a complaint of stomach pain

CHIEF  COMPLAINT: A 14 yr old male patient came to causality  with a complaint of stomach pain since 15 days
HISTORY OF PRESENT ILLNESS  : A  14 yr old male patient who is student by occupation  was apparently asymptomatic  15 days back then he noticed continous dragging  type of pain in the abdomen.
First patient visited local hospital  and then came to  causality  
HISTORY OF PAST ILLNESS  :  
History of similar  complaints 5 months ago
H/o chicken pox 1year back 
No h/o hypertension  diabetes  epilepsy  asthma  thyroid  renal and cardiac problems 
FAMILY HISTORY  : 
Similar complaints of his sister who passed away 
PERSONAL HISTORY  : 
Unmarried 
Diet :mixed
Appetite : normal 
Sleep : adequate 
Bowel  and bladder movements  : regular
Micturation : normal
No addictions 
No known history of drug allergy
GENERAL EXAMINATION:  patient  is conscious coherent cooperative and well oriented to time place and person 
No h/o pallor  cyanosis  clubbing bilateral
  pedal edema  
H/o  Edema of abdomen 
Vitals :
Temp : afebrile 
BP: 110 /70 
PP :82
RR :16
SYSTEMIC EXAMINATION  :
CVS: no thrills and cardiac murmurs
S1s2 heart sounds heard
RS : no wheeze dysponea 
Position of trachea : central 
Abdomen :
Shape : schapoid
Liver not palpable 
Spleen palpable 
Bowel sounds : heard( 12 )
CNS : conscious
Speech normal
No neck stiffness 
PROVISIONAL DIAGNOSIS  :
Splenomegaly

INVESTIGATIONS 
ECG 

serum creatine: 0.6mg/dl(0.5 to 1)
Blood urea : 17mg /dl(12- 42)

A 40 year old male patient with complaint of weakness in right upper limb
