CHEIF COMPLAINT :
A 21 yr old female patient came to the causality with a complaint stomach pain and vomitings since a day
HISTORY OF PRESENT ILLNESS:
A 21 yr old female patient who is student by occupation was apparently asymptomatic then she noticed continuous stomach pain (on menstruation)and had 8 to 9 episodes of vomiting
PAST MEDICAL HISTORY:
H/o appendix surgery
PERSONAL HISTORY :

Sleep : adequate
Appetite : normal
Bowel and bladder movements : regular
Micturation : normal
Menstrual history : regular
GENERAL EXAMINATION:
patient is conscious coherent cooperative and well oriented to time place and person
No h/o pallor cyanosis clubbing edema
Icterus lympadenopathy

Gait : normal
SYSTEMIC EXAMINATION :
CVS : no thrills
No cardiac murmurs
Cardiac sounds : s1 s2 +

Tenderness present in umbilical region
